midnight, obama care. for the first time every american expected to have health insurance, 50 million uninsured will be able to start shopping tomorrow. paula faris tells us what to expect when you check in. >> reporter: max was dropped from his health iurance because of his pre-existing condition. >> i have asma and allergies. >> courtney says she can't afford it. >> i need good maternity coverage. >> reporter: we heard from so many of you. >> will i have more affordable health care? >> reporter: all part of the 48 million americans who do not have insurance, about 15 percent of the population. >> how do i sign up and when does the coverage begin. >> reporter: starting at midnight you can go on health care.gov where you'll be able to shop around, compare plans and buy. everyone regardless of age, gender, income or pre-existing condition will be able to get insurance. if you don't want to buy or have been delaying because of

expense, you'll have until march 31st, six months to purchase a plan or face a fine from the government. >> i just want to be covered like anybody else. >> reporter: no one will know how good of a deal this is until midnight. but for people like max, he can breathe easier now knowing he has a choice. >> i just want to have that safety net. >> reporter: that is the big mystery. we will not know the details of these plans or how much they cost until midnight. tomorrow we're walking folks through it and really helping them understand who they can trust throughout the process. >> we'll be doing it right here and making it simple. thank you, paula. there is something happens all around you tonight proving that american health care costs have been out of control far and away the highest in the world. so people are taking their business abroad tonight. abc's byron pitts got on a plane with some of the 1 million

americans who are in a sense paid by their employers to go on a surgical vacation. >> reporter: for 39-year-old joy gulon of hickory, north carolina. this is her first time on a plane. >> reporter: her destination? the sun soaked south american hot spot, costa rica. a four star hotel. local driver, personal concierge. but a full fledged vacation it's not. joy is a medical tourist. she's come to costa rica for wright loss surgery because of a family history of diabetes and heart disease. >> could you have afforded this at home? >> no. >> no chance? >> no. >> reporter: and what's more, her company is paying the entire bill. they gave joy as choice. a traditional insurance plan or out sourcing her care in a foreign country. >> out of pocket, how much we have t >> nothing, zero. >> reporter: back home joy's gastric sleeve surgery goes for $30,000.

here, less than $18,000. her company, hsm, a furniture manufacturer in western, nc tells abc news it has saved nearly $10 million in health care cost the past 5 years by sending close to 250 employees abroad for medical procedures. another hsm employee on the costa rica trip is 65-year old gary harwell. a retired plant manager, gary's going for knee replacement. his doctor who earned his medical degree in latin america has received advanced training in the u.s. in the u.s. gary's knee replacement would have cost more than $50,000. in costa rica it's half that. in the u.s. gary and joy would have each paid $3,000 out of pocket. in costa rica, nothing. get this, when the bandages come off, joy and gary will get a bonus check for at least $2,500 from their company. a percentage of the corporate savings in insurance costs.

but medical experts who are tracking the increasing number of americans going abroad for medical care are concerned. >> what are the biggest challenges? >> risk of infection. risk of a poorly performed procedure. >> reporter: at this private hospital there has been a very low rate of problems. for patients like joy, the opportunity outweighs the risk, the opportunity of a better, healthier life without breaking the bank. byron pitts, abc news. we move on. tonight we are learning about an extraordinary decision in the american military, two marine generals were fired for negligence, for failing to protect their troops and equipment at a large base in southern afghanistan. two marines were killed when the base was attacked by the taliban last year, the first time since the vietnam war that a general

was fired for negligence after an enemy attack. next to chicago where investigators are searching for the mysterious caution of this collision with a run away train. an empty train slammed into a commuter train loaded with passengers. dozens were hurt. the morning rush hour came to a halt. the empty train has been in a repair yard when somehow it began rolling down the tracks. another bizarre incidents of road rage yesterday, motorcycle kl c occurreding a car with a dad. >> reporter: watch as 30 bikers take over this new york hare, surrounding this black range rover. look, one of the riders seems to stop short. the suv hits him, breaking his leg. within seconds the entire group grinds to a halt. they surround the suv police say attacking the car and slashing

its tires. that the when the suv takes off, plowing over some of the bikers. watch again. police say the driver in the car with his wife and young daughter was afraid for his life. then a high speed chase. the bikers in hot pursuit. at one point they catch up. one biker tries to open the car door. but again it speeds away. the chase ends on the streets of manhattan. the driver trapped in traffic. the bikers surround the car again. this one smashing the driver's side window with his helmet. >> he's taken out of the car. he is assaulted. he received some stitches at columbia presbyterian hospital. >> reporter: tonight police are trying to track down the bikers and pouring over every second of that video. now the news tonight on the american student, amanda knox back in the spotlight once again on trial for the murder of her roommate. an appeals court in italy is

again holding out the threat that she could be forced back to italy and back to jail. abc's gio benitez with the latest on what is happening to her tonight. >> reporter: as her third murder trial kicks off in italy, a world away in seattle, amanda knox says she's living a nightmare. >> the longer this plays out the longer it's going to take me to get my life back and to move on. >> reporter: it's been two years since knox returned home. after an italian appeals court overturned her murder conviction. she and her then boyfriend were accused of killing knox's roomate meredith kercher. back in the u.s. knox reenrolled in college, found a new boyfriend, and wrote a book. but then this spring, a crushing blow. the italian supreme court ordering a retrial. knox told diane, it was incredibly painful news. >> i felt like, after crawling
